A Ledger recovery phrase backup should be created during initialization by following the hardware screen, writing the words in order, checking spelling, and keeping the record offline. It is not a daily login note and should not be stored together with the device PIN.
Understand what the recovery phrase is before writing

Write each word from the device screen in order
During initialization, watch the hardware screen and write each word carefully in the shown order. Do not guess from the app page or copy the words into a computer or phone. If this is part of first setup, compare the sequence with the first-time initialization flow.
Separate PIN, app lock, and phrase records

Review the backup without copying the words
